To understand why cortisol has become the villain of the wellness world, one must first understand its biological function. Produced by the adrenal glands—small, triangular organs sitting atop the kidneys—cortisol is a steroid hormone that belongs to a class of chemicals known as glucocorticoids. Its production is governed by the hypothalamic-pituitary-adrenal (HPA) axis, a complex feedback loop that links the brain to the endocrine system. When the brain perceives a threat—whether it is a looming work deadline or a physical predator—the hypothalamus releases corticotropin-releasing hormone (CRH), which signals the pituitary gland to secrete adrenocorticotropic hormone (ACTH), which finally prompts the adrenal glands to pump cortisol into the bloodstream.
“Healthy levels of cortisol have beneficial effects,” says Sophie Trotman, a nutritionist and workplace wellbeing consultant. The hormone plays a key role in vital bodily functions ranging from regulating your metabolism to keeping your circadian rhythm on the rails. In a healthy system, cortisol levels follow a diurnal rhythm: they peak in the early morning to help pull us out of sleep and provide the energy needed to start the day—a phenomenon known as the Cortisol Awakening Response (CAR)—and gradually decline throughout the evening to allow for the onset of melatonin and restful sleep. “It also has anti-inflammatory properties, which can help in the management of minor injuries or immune responses,” Trotman says, adding that it can “enhance focus, memory, and resilience, making it easy to perform well under pressure.”
However, the "fight or flight" mechanism that cortisol facilitates was evolutionarily designed for short-term bursts of activity. In the modern world, the "threats" we face are often chronic rather than acute. Instead of running from a predator for ten minutes, we worry about our mortgages for ten years. This persistent activation of the HPA axis leads to a state of chronic hypercortisolemia, where the body is perpetually flooded with the hormone. According to Dr. Chandni Rajani, GP and founder of her namesake clinic, "stress, poor sleep quality, and quantity can increase cortisol production." When the body stays in this high-alert state, the very hormone designed to protect us begins to cause systemic damage.
The physical manifestations of chronic high cortisol are varied and often insidious. One of the most common complaints linked to the hormone is "stress-induced weight gain." Cortisol stimulates glucose production, providing a quick energy source for the muscles. However, if that energy isn’t used through physical exertion, the body stores it as fat—specifically visceral fat around the midsection. This type of fat is metabolically active and can lead to further inflammation and insulin resistance. Dr. Daniel Masud, Consultant General & Addictions Psychiatrist at Schoen Clinic Chelsea, notes that high cortisol can be a contributing factor to "high blood pressure and heart issues, digestive issues, skin and hair changes, lowered immune response, and reduced libido and fertility issues."
Beyond the physical, the mental toll of cortisol dysregulation is profound. Dr. Elena Touroni, a consultant psychologist and co-founder of the Chelsea Psychology Clinic, warns that if your cortisol levels are high for an extended period, you put yourself at risk of “impaired memory and focus.” She explains that “chronic high cortisol is also linked to mood disorders like anxiety and depression. Managing cortisol helps maintain a balanced mood, better energy levels, and a healthier body, contributing to improved resilience and quality of life.” When the brain is constantly bathed in stress hormones, the hippocampus—the area of the brain responsible for memory and emotional regulation—can actually begin to atrophy, making it harder to handle future stress.
So, what is the trick to managing your cortisol in a world that seems designed to spike it? The solution lies in a multi-pronged approach involving exercise, diet, sleep hygiene, and psychological interventions.
When it comes to managing cortisol, physical exercise and rest are the biggest tools in your arsenal. However, there is a paradox at play. “High cortisol is associated with weight gain and muscle loss, both of which can hinder fitness goals,” says Dr. Masud. Furthermore, the type of exercise matters immensely. “Exercise is one of the most effective ways to help to regulate cortisol levels,” Trotman says, but she cautions that “high-intensity or excessive exercise may temporarily raise cortisol levels. If you’re already going through a stressful season of life, this may be problematic. Listen to your body.” For someone already suffering from burnout, a punishing 45-minute HIIT (High-Intensity Interval Training) session might do more harm than good by stacking more physiological stress onto an already burdened system. Instead, Masud notes that many are finding success with “interval training, yoga, and outdoor activities, which have been shown to promote relaxation and reduce cortisol.”
In addition to movement, the role of mindfulness cannot be overstated. The goal of mindfulness is to stimulate the parasympathetic nervous system—the "rest and digest" counterpart to the "fight or flight" system. “Meditation, deep breathing, and mindfulness practices help activate the body’s relaxation response, counteracting stress,” says Masud. Even five to ten minutes of controlled diaphragmatic breathing can signal to the brain that the "threat" has passed, allowing cortisol levels to begin their descent. For those who find traditional meditation daunting, digital tools like Headspace or Calm offer guided sessions that make the practice accessible.
The intersection of technology and health has also provided new ways to monitor these fluctuations. Wearable technology, such as smartwatches, Whoop bands, and Oura rings, has become increasingly sophisticated. These devices track heart rate variability (HRV), sleep stages, and skin temperature, providing a proxy for how the autonomic nervous system is functioning. As Dr. Masud points out, these devices "can provide insights into cortisol fluctuations and help users optimize lifestyle choices." By seeing how a late-night meal or a stressful meeting affects their "readiness" or "recovery" scores, individuals can make data-driven adjustments to their routines.
Dietary choices play an equally critical role in hormonal stabilization. “A balanced diet rich in whole foods, antioxidants, and healthy fats, combined with good hydration, can further support hormonal balance,” says Dr. Rajani. High-sugar diets are particularly problematic; sugar spikes cause insulin to surge, which can subsequently trigger a cortisol release as the body tries to stabilize blood sugar levels. This creates a vicious cycle of cravings and stress. To combat this, Dr. Masud recommends a diet rich in “lean protein and omega-3 fatty acids from sources like salmon, walnuts, and flaxseed.” Trotman also highlights the importance of micronutrients: “Nutrients like magnesium and vitamin C, found in foods such as leafy greens and citrus fruits, may help buffer cortisol responses and support adrenal health.” Magnesium, in particular, is often called "nature’s relaxant" due to its ability to regulate the neurotransmitters that calm the nervous system.
Finally, we must address the "elephant in the room" created by social media: the concept of "cortisol face." On platforms like TikTok and X (formerly Twitter), influencers have popularized the idea that elevated cortisol is responsible for making people "ugly" by causing facial puffiness and premature aging. While there is a grain of truth in the idea that chronic stress affects appearance, experts urge caution. “While cortisol can impact facial features, the social media ‘cortisol face’ is an oversimplification of what’s actually going on,” says Dr. Daniel Careaga, MD, a double board-certified plastic surgeon. He notes that while extreme clinical conditions like Cushing’s Syndrome can cause a "moon face" appearance due to excessive cortisol production, everyday stress is unlikely to cause permanent structural changes. “In fact, temporary puffiness is most commonly linked to fluid retention caused by a lack of sleep, sodium-rich food and drink intake, alcohol consumption, allergies, or hormonal changes,” Dr. Careaga explains.
Ultimately, the goal of cortisol management is not to achieve a "zero-stress" life—which is both impossible and counterproductive—but to foster a resilient nervous system. A body that can spike cortisol when needed and then efficiently return to a baseline of calm is a healthy body. By prioritizing sleep, choosing the right forms of movement, nourishing the body with anti-inflammatory foods, and using technology as a guide rather than a source of further anxiety, we can transform cortisol from a feared enemy into the vital ally it was meant to be. The "stress hormone" is simply a messenger; if we listen to what it’s telling us about our lifestyle, we can find the balance necessary for long-term health and vitality.
