As the former director of the FDA’s Center for Food Safety and Applied Nutrition (CFSAN) from 2015 to 2023, I witnessed firsthand how the success of nutritional initiatives is tethered to the integrity of our food safety systems. We cannot effectively encourage the public to adopt a diet rich in fresh, "real" foods if those very items pose an acute risk of debilitating illness. In mid-2026, this tension has reached a boiling point as the United States grapples with a record-breaking surge in cyclosporiasis cases, highlighting a systemic vulnerability that threatens to undo years of nutritional progress.
Currently, the Food and Drug Administration is managing the investigation of six distinct outbreaks of Cyclospora cayetanensis, a microscopic parasite that causes a severe intestinal illness known as cyclosporiasis. While one of these outbreaks has been definitively linked to shredded iceberg lettuce, the sources for the remaining five remain unidentified, casting a shadow of uncertainty over the produce aisle. This parasite is particularly insidious because it is often associated with fresh fruits and vegetables—the very "real foods" that nutritionists and public health advocates have spent decades promoting as the foundation of a healthy lifestyle. Unlike many bacterial pathogens that can be killed through cooking, Cyclospora is frequently found on items intended for raw consumption, such as cilantro, raspberries, and leafy greens, making it a unique challenge for both regulators and consumers.
The psychological impact of these outbreaks on consumer behavior cannot be overstated. When a foodborne illness outbreak makes headlines, the public’s reaction is often a swift and defensive pivot toward ultra-processed foods. In recent weeks, social media platforms have been inundated with a peculiar brand of gallows humor, with users posting comments like "more pizza!" and "Oreos never gave me explosive diarrhea!" This sentiment reflects a dangerous "processed food safety paradox." Consumers perceive shelf-stable, highly processed goods—which undergo extreme heat treatment and contain preservatives—as "safer" than a fresh salad. I have heard consumers state they would rather eat a bag of Doritos than a head of romaine lettuce during an E. coli scare. While a bag of chips might not carry a risk of immediate parasitic infection, the long-term health consequences of shifting away from fresh produce toward high-sodium, high-sugar processed foods are catastrophic, contributing to the rising tide of obesity, Type 2 diabetes, and cardiovascular disease.
To address these risks, the FDA has historically utilized a data-driven approach to identify which foods are most susceptible to microbial contamination. Under the congressionally mandated FDA Food Safety Modernization Act (FSMA), the agency developed the "Food Traceability List" (FTL). This list identifies high-risk commodities that require additional record-keeping to facilitate rapid traceback during an outbreak. The FTL includes cucumbers, fresh-cut fruits, herbs, leafy greens, melons, peppers, sprouts, tomatoes, and certain cheeses. These items share a commonality: they are nutritional powerhouses. While chemical safety is undeniably important, it must not come at the cost of the infrastructure required to prevent acute foodborne illness. Microbial pathogens work much faster than chemical toxins; a single contaminated batch of lettuce can sicken thousands in days, whereas chemical exposure often manifests over decades. The current administration has taken several steps that arguably undermine the microbial safety system, including budget reallocations and a softening of inspection frequencies for high-risk produce facilities. This cumulative erosion of oversight creates a "diminished food safety system" where the focus is fragmented rather than holistic.
Compounding these regulatory challenges is the administration’s public skepticism regarding climate change, which has been labeled a "hoax" in various political spheres. From a scientific and food safety perspective, this denialism is dangerous. There is an established and growing body of evidence linking environmental shifts to increased foodborne illness risks. Warming global temperatures allow parasites like Cyclospora to survive and proliferate in environments where they were previously rare. Furthermore, extreme weather events, such as heavy rainfall and flooding, lead to significant runoff from livestock operations into irrigation water sources. This contaminated water then introduces pathogens directly onto the crops. When the climate changes, the geography of risk changes, and a failure to account for these environmental variables leaves our food supply chain exposed.
The environmental cost of these outbreaks also extends to the global issue of food waste. When a large-scale outbreak is detected, the standard response is the mass destruction of the suspected product. We are currently seeing the destruction of massive quantities of lettuce and other produce, much of it imported from international partners like Mexico. This represents a staggering waste of energy and resources. Consider the energy expended in the original cultivation, the water used for irrigation, the fuel for international distribution, and the electricity for cold storage—all of it rendered moot when the product is sent to a landfill. We are at a juncture where the "Food is Medicine" movement could be derailed by a lack of confidence in the "medicine" itself. If consumers cannot trust that a piece of fruit or a bowl of greens is safe, they will return to the perceived "safety" of the middle aisles of the grocery store, where nutrition is often sacrificed for shelf-life and sterility. To restore this confidence, the federal government must recommit to a robust microbial safety agenda that recognizes the unique challenges of the 21st-century supply chain.
