The crux of the dispute involves the official classification of measles-related deaths, a technical determination that has historically been left to state medical examiners and epidemiologists but has now become a flashpoint for political maneuvering. In the landscape of infectious disease management, the last thing public health infrastructure needs is administrative drama; the focus must remain steadfastly on responding to the outbreak itself and preventing further loss of life.

This system, governed by established case definitions, is the backbone of national epidemiological surveillance. Developing these definitions is typically a long, deliberative, and highly technical process. Without trust, the nation’s ability to respond effectively to an outbreak is compromised, as public cooperation with vaccination drives and quarantine measures depends on the perceived integrity of the information provided.
When infectious disease outbreaks occur, states typically rely on the CDC for specialized assistance. This support includes access to high-complexity laboratory testing, assistance with the logistical nightmare of contact tracing, data management support, and consultation on infection-control protocols. In more severe scenarios, states may request an "Epi-Aid"—an on-the-ground deployment of the CDC’s Epidemic Intelligence Service (EIS). Known as the nation’s "disease detectives," these officers provide the frontline expertise needed to identify the source of an outbreak and stop its spread.
This collaborative model was on full display in 2025 during an outbreak in Texas. At that time, Texas reported the nation’s first measles-related death in a decade, which was followed shortly by a second fatality. Throughout that period, the protocol remained professional and predictable. Texas state officials investigated the cases, made the clinical and epidemiological determinations, and notified the CDC. The federal agency then incorporated those deaths into its national reporting dashboard without delay or public questioning. Furthermore, the CDC provided Texas with significant resources, including financial support, emergency vaccine supplies, and personnel to assist with air-handling system consultations in hospitals and the implementation of infection-control protocols.
These tragic deaths in Texas were a cause for alarm, but from a clinical perspective, they were not unexpected. Measles is a severe respiratory virus, and historical data indicates that approximately 1 to 3 out of every 1,000 children who contract the infection will succumb to it or its complications. What was unexpected, however, was the emergence of an active campaign by fringe groups, such as Children’s Health Defense, to cast doubt on the cause of these deaths. These organizations attempted to shift the blame away from the virus and onto the medical professionals and hospitals that treated the children, effectively exploiting grieving families to push an anti-vaccine agenda.
The situation was further complicated by the public actions of the Health Secretary. In a notable appearance on national television, the Secretary was seen consuming fast food while making scientifically unfounded claims regarding the Measles, Mumps, and Rubella (MMR) vaccine. He falsely suggested that the vaccine causes more fatalities than the virus itself and claimed that the protection offered by the vaccine wanes to the point of being ineffective. Both claims have been repeatedly debunked by decades of peer-reviewed research and global health data.
During our tenure at the CDC, as these events unfolded, we worked tirelessly with career scientists to provide accurate scientific analyses. We attempted to route these corrections through the agency’s political leadership to counter the misinformation being disseminated by the Secretary. An outbreak is the worst possible time for the nation’s health leaders to spread disinformation. Despite our offers to brief the Secretary on the fundamentals of immunology and childhood vaccines, such a meeting was never granted. Instead, the misinformation continued, with the Secretary using social media to promote "healers" who claimed that inhaled steroids and antibiotics could treat measles—treatments for which there is no supporting evidence. He also attempted to market Vitamin A as a miracle cure or a preventative measure, ignoring the nuances of medical supervision.
The pressure from the Department of Health and Human Services (HHS) eventually escalated to a request for the CDC to revise its official clinical guidance. They wanted to include these unproven and potentially harmful treatments in the recommendations for treating children with measles. We declined to endorse these changes. Steroids and antibiotics are not routine treatments for a viral infection like measles and lack the "gold-standard" evidence required for CDC endorsement. In August, Pennsylvania’s governor announced two deaths related to the ongoing measles outbreak. The state followed the exact protocol used by Texas in 2025. Yet, this time, the CDC’s political leadership publicly questioned the state’s findings in a social media post and refused to add the deaths to the national tracking dashboard. The only discernible difference between the two situations appears to be the political affiliation of the state’s leadership and the governor’s vocal criticism of the current administration.
This disparity raises a fundamental question about the future of American public health: Is the CDC now applying different standards to data based on the political landscape of the state in question? At a time when measles cases are rising and vaccination rates among kindergartners are falling to dangerous levels, the role of the CDC Director should be to serve as a trusted voice of science. Instead, the newly confirmed director, Erica Schwartz, has withheld Pennsylvania’s reported deaths from the national count. This decision persists despite reports from outlets like The Atlantic, where the parents of a deceased infant have stated their belief that measles was the cause of their child’s death. It is unclear why the CDC leadership feels the need to adjudicate these deaths from a distance rather than working collaboratively with the state’s medical experts.
The human cost of this political infighting is immense. In Pennsylvania alone, over 400 people have been infected, with 83 hospitalizations reported. Measles is not a mild childhood illness; it can lead to pneumonia, permanent hearing loss, and encephalitis (swelling of the brain). It is also known to cause "immune amnesia," a condition where the virus wipes out the body’s existing immune memory, leaving children vulnerable to other infectious diseases for years. In rare and tragic cases, the virus can lead to Subacute Sclerosing Panencephalitis (SSPE), a fatal neurological disorder that emerges years after the initial infection.
The epicenter of the current Pennsylvania outbreak is Lancaster, the very location where the Secretary of Health previously claimed that chicken noodle soup and Vitamin A were sufficient substitutes for vaccination. Such claims are not just incorrect; they are dangerous. The most effective way to protect the nation’s children is not a mystery: it requires providing clear, evidence-based information and ensuring that families have easy access to vaccines.
The CDC must return to the principles that once made it the world’s premier public health agency. It must report data consistently, work in good faith with state partners, and advocate for treatments and preventative measures grounded in rigorous evidence. Public health data is a national resource that should never be manipulated for political leverage. For the safety of the country, these metrics must be reliable, transparent, and—as boring as it may seem—free from the influence of partisan politics. Only then can we effectively combat the resurgence of preventable diseases and protect the most vulnerable members of our society.
